Our Care Team


Nursing

Our living accommodations are supported by caring staff who go above and beyond, providing holistic care and promoting resident independence with safety always in mind. The needs of our residents are met with patience and understanding of their unique physical and mental health diagnoses and the struggles that can come as a result. 

Our nursing care team consists of Registered Nurses (RNs), Licensed Practical Nurses (LPNs) and certified Health Care Aides (HCAs). With around the clock 24/7 care, RNs and LPNs are assigned as case managers to each resident and oversee managing all aspects of their care. As case managers, they collaborate with the interdisciplinary team and liaise with physicians, psychiatrists, and other external healthcare professionals in maintaining or improving a resident’s health status. 

We recognize the importance of regular updates and communication with family members and legal/public guardians as an important piece of person-centered care. To this end, our RN and LPN team will reach out regarding changes in a resident’s health status or any other significant events so you can rest assured that your loved one is being looked after. 

Our Nursing Management (Clinical Lead and Care Coordinator) collaborates closely with AHS Transition Services in the transfer and admission of residents to our facility. This includes an assessment and screening procedure to ensure that the facility can meet the complex needs of our clientele and so that the resident can settle successfully into living within the care center. 

In the case of acute illness, the physician may request that the resident be transferred to an acute care hospital for medical treatment. While away from the facility for whatever reason, the resident remains responsible for accommodation fees and other resident charges.

In the event that Glamorgan Care Centre is unable to safely and/or appropriately meet the care needs of a resident, the Case Managers will initiate discharge planning with the participation of the legal guardian, family as appropriate, the care team, physician, psychiatrist and AHS Transition Services. From this process, the resident may be transferred to another continuing care center, acute care hospital or specialized care placement that can better meet the unique needs of the resident.


Mental Health

The facility is supported by general practitioners, who have long term care privilege, and psychiatrists affiliated with Alberta Health Services. Every resident is provided a general practitioner and psychiatrist for both physical and mental health needs. Psychiatric rounds with our psychiatrists are completed monthly. These rounds or meetings are also attended by a Pharmacist, Legal/Public Guardian, the Glamorgan interdisciplinary team and the resident and/or family member as appropriate. This team reviews changes to the resident’s mental health baseline, general mental health status and psychiatric medications. 
Glamorgan Care Centre also has access to 2 external resources for behavior management: 
  • ABC Team (Applied Behavior Collaborative Team) 
  • ICM (Intensive Case Management) in the community 
​These resources support the residents and staff by ensuring that appropriate care is being maintained, behavior management and education is being followed, and assisting the facility in transfers for residents whose complex or fluctuating needs outgrow the capabilities of the facility. 
Glamorgan Care Centre is also partnered with the Canadian Mental Health Association (CMHA). CMHA offers clinical mental health services and leisure/recreational activities that provide opportunities for residents to connect and be more active in the community.

Interdisciplinary Team

Glamorgan Care Centre’s management consists of an interdisciplinary team including a Clinical Lead, Care Coordinator, Clinical Educator, Executive Director, and Director of Recreation. This team works together in evaluating resident care outcomes, providing employee support, direction, education, and supervision. Through inter-collaborative leadership, the facility’s daily operations and high standards of care are achieved and maintained. This team is also on-call after hours for further support if needed 
Glamorgan holds regular interdisciplinary team meetings to discuss the overall and changing care needs of our residents. These meetings include daily shift reports, weekly interdisciplinary intake, monthly risk management, in-house psychiatric visits, annual care conferences and as-needed, special care conferences.

Recreation

Recreation and leisure opportunities are available Monday- Saturday for all residents. 

All residents are assessed upon admission to determine the activities that are of greatest interest and benefit to them. Ongoing evaluation of resident participation and program effectiveness is done to ensure optimal services are being offered and that residents are benefitting from them. 

A monthly program calendar is prepared each month to inform residents, family members, and staff of the upcoming events and activities. Outings are of special interest to our residents. Careful consideration and planning are taken to ensure outings are affordable for all residents. Residents who are financially unable to afford certain outings are supported by resident funds raised by the Recreation Department. 

​The Recreational Therapist lead our Resident & Family Council Meeting held every first Tuesday of each month. This meeting is designed for the residents and families to have an opportunity to voice their opinions, concerns, and suggestions regarding their life at Glamorgan Care Centre. Meeting minutes are posted on the recreation board located in the East hallway.


Food Services

A Registered Dietitian is available to assess, develop specialized nutrition plans for residents with special therapeutic dietary requirements and works closely with the head cook. Special textured diets and swallowing assessments are provided. 

​Three appetizing daily meals are provided to residents by the Glamorgan Food Services Department. A three-week rotating menu based on Canada’s Food Guide is offered and the menu is changed every six months to reflect seasonal changes based on the quality the ingredients we have access to. Resident’s preferences and dislikes and therapeutic dietary needs are taken into consideration. Cultural and religious factors are respected. A selection of beverages and light snacks are provided in between meals and available at all times for residents.


Pharmacy

Professional pharmacy services are provided to Glamorgan by Alberta Health Services contract pharmacy, guided by appropriate reginal processes and Alberta Health Services Formulary. Clinical pharmacy consultation is available and provided during care conferences, Psychiatrist rounds and on an as needed basis. Complete medication reviews are conducted every 3 months by the Pharmacy and Professional Nurse with the residents' physician. 
Glamorgan nursing staff may only administer prepackaged medications that have been provided by our contracted pharmacy or medications with a Drug Identification Number (DIN) on the package and approved by the physician. Any over the counter medications or herbal remedies must be discussed with the Professional Nurse, pharmacist and the physician before it is administered to the resident

Diagnostic Services

The Calgary Laboratory Services does routine collection of blood weekly. Additional collection for high priority and emergent situations are done on an as needed basis. Community based diagnostic imaging are used as required for x-ray, ultrasound, mammogram or any other services.

Laundry

Laundry services are available 7 days/week with monthly charge as indicated in the Resident and Family Handbook. 

​In order to avoid confusion about ownership, all personal clothing items must be labeled upon admission, and whenever new clothing items are brought to the care centre. The Laundry Service staff will label all clothing to minimize the chance of loss of any articles. 
All personal laundry is done at Glamorgan. Bedding and larger towels are transferred to Mayfair Care Centre twice a day.

Smoking

Glamorgan Care Centre has a smoking/intake committee who meets regularly and as needed to review and care plan over incidents of safe and unsafe smoking. A smoking coordinator is also available to supervise all smoking activities in the care centre. 
The facility provides a designated outdoor smoking area for the residents. Residents are assessed on their safety to smoke independently an ongoing basis. Any resident deemed as unsafe smoker is put under the supervision of the smoke chief. Unsafe smoking is a risk to all residents and staff and is therefore not tolerated. 

​Smoking cessation assistance is available to residents if they so desire and is coordinated with the physician and pharmacy.

Environmental Services

Our Housekeeping & Laundry team are dedicated to the processes used to maintain and enhance the living spaces of our Resident's. Following Infection Prevention and Control measures, AHS standards and using the latest in technology, our home is maintained daily. Monthly and Annual mini and deep cleaning is performed in conjunction with our Medical team to further enhance the cleanliness and sanitation of all spaces throughout the home. All linens and Resident clothing are washed in-house and delivered back to our resident rooms within 24 hours. ​
